Safe custody.—Every employer of a maternity beneficiary shall keep every declaration form to which these rules apply in a receptacle or place reserved for the purpose at the premises where the employment of women entitled to maternity benefit is carried on, shall record in Form 5 the description of every declaration form so kept and the date on which it was received, and shall furnish a copy of the record to the Regional Board within ten days of the close of each year.
