Surrender of certificate.—Every employer of a maternity beneficiary who ceases to carry on the employment of women entitled to maternity benefit shall, within fifteen days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 2 of the manner in which every declaration form held by him has been disposed of.
