Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the ambulance operator shall apply to the State Medical Council in Form 28 for a duplicate, accompanied by a fee of two thousand rupees, and the State Medical Council shall issue a duplicate certificate marked as such within twenty-one days of the application.
