Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the establishment employing persons with disabilities shall apply to the Government of Uttarakhand in Form II for a duplicate, accompanied by a fee of one hundred rupees, and the Government of Uttarakhand shall issue a duplicate certificate marked as such within thirty days of the application.
