Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the nursing home shall apply to the District Registering Authority in Form 21 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the District Registering Authority shall issue a duplicate certificate marked as such within forty-five days of the application.
