Particulars of application.—Every application for registration shall state the name and address of the applicant, the premises at which the carriage of goods for hire is carried on, the particulars of every pollution certificate held by the applicant and such other particulars as are specified in Form 23, and shall be signed by the applicant or his authorised agent.
