Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the rehabilitation professional shall apply to the Commission in Form V for a duplicate, accompanied by a fee of ten rupees, and the Commission shall issue a duplicate certificate marked as such within twenty-one days of the application.
