Regulation 24, National Medical Commission (Licensing of Pharmacists) Regulations, 2021 [S9Q-95M]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the pharmacist shall apply to the District Registering Authority in Form 28 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the District Registering Authority shall issue a duplicate certificate marked as such within twenty-one days of the application.
