Surrender of certificate.—Every insurance medical practitioner who ceases to carry on the provision of medical benefit shall, within thirty days of such cessation, surrender the certificate of registration to the Regional Board in person or by registered post, and shall inform it in Form 6 of the manner in which every wage record held by him has been disposed of.
