Intimation of change.—Every insurance medical practitioner shall intimate to the Regional Board, in Form 2 and within fifteen days of the change, any change in the particulars furnished in the application for registration, and the Regional Board shall amend the certificate accordingly on payment of a fee of five hundred rupees.
