Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the immediate employer shall apply to the Standing Committee in Form 6 for a duplicate, accompanied by a fee of one hundred rupees, and the Standing Committee shall issue a duplicate certificate marked as such within ninety days of the application.
