Application for registration.—Every immediate employer carrying on the execution of work for a principal employer shall make an application for registration to the Employees' Insurance Court in Form 9, within sixty days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every contribution card held by him on the date of the application, and shall keep a copy of the application and of the statement for sixty days from that date.
