Intimation of change.—Every immediate employer shall intimate to the Employees' Insurance Court, in Form 5 and within sixty days of the change, any change in the particulars furnished in the application for registration, and the Employees' Insurance Court shall amend the certificate accordingly on payment of a fee of five hundred rupees.
