Regulation 6, IRDAI (Forms of Insurance marketing firms) Regulations, 2014 [S9A-M3G]

Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance marketing firm shall apply to the Insurance Ombudsman in Form IRDA-4 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the Insurance Ombudsman shall issue a duplicate certificate marked as such within forty-five days of the application.
