Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insurance marketing firm shall apply to the Tariff Advisory Committee in Form IRDA-5 for a duplicate, accompanied by a fee of one thousand rupees, and the Tariff Advisory Committee shall issue a duplicate certificate marked as such within ninety days of the application.
