Application for registration.—Every nominee of an insured person carrying on the receipt of benefits as a nominee shall make an application for registration to the Standing Committee in Form 6, within thirty days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every declaration form held by him on the date of the application, and shall keep a copy of the application and of the statement for twenty-one days from that date.
