Application for registration.—Every certifying medical authority carrying on the certification of disability shall make an application for registration to the Government of Tamil Nadu in Form V, within fifteen days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every identity card held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
