Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the certifying medical authority shall apply to the Government of Tamil Nadu in Form III for a duplicate, accompanied by a fee of two hundred rupees, and the Government of Tamil Nadu shall issue a duplicate certificate marked as such within fifteen days of the application.
