Surrender of certificate.—Every contractor for the mid-day meal who ceases to carry on the supply of mid-day meals shall, within twenty-one days of such cessation, surrender the certificate of registration to the Government of Uttar Pradesh in person or by registered post, and shall inform it in Form D of the manner in which every attendance register held by him has been disposed of.
