Application for registration.—Every contractor for the mid-day meal carrying on the supply of mid-day meals shall make an application for registration to the Government of Uttar Pradesh in Form E, within sixty days of the commencement of these rules, accompanied by a fee of twenty-five thousand rupees and a statement of every certificate of recognition held by him on the date of the application, and shall keep a copy of the application and of the statement for seven days from that date.
