Application for registration.—Every anganwadi centre carrying on the running of an anganwadi centre shall make an application for registration to the Committee in Form V, within thirty days of the commencement of these rules, accompanied by a fee of five hundred rupees and a statement of every institution held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
