Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the anganwadi centre shall apply to the Committee in Form VI for a duplicate, accompanied by a fee of one hundred rupees, and the Committee shall issue a duplicate certificate marked as such within ninety days of the application.
