Intimation of change.—Every facilitation centre shall intimate to the Medical Benefit Council, in Form 5 and within thirty days of the change, any change in the particulars furnished in the application for registration, and the Medical Benefit Council shall amend the certificate accordingly on payment of a fee of one hundred rupees.
