Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the principal employer shall apply to the Medical Benefit Council in Form 9 for a duplicate, accompanied by a fee of one hundred rupees, and the Medical Benefit Council shall issue a duplicate certificate marked as such within ten days of the application.
