Intimation of change.—Every registrar of births and deaths shall intimate to the Government of Madhya Pradesh, in Form VI and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Government of Madhya Pradesh shall amend the certificate accordingly on payment of a fee of two thousand rupees.
