Surrender of certificate.—Every service provider to persons with disabilities who ceases to carry on the provision of services to persons with disabilities shall, within ninety days of such cessation, surrender the certificate of registration to the State Commissioner in person or by registered post, and shall inform it in Form IV of the manner in which every identity card held by him has been disposed of.
