Surrender of certificate.—Every hospital who ceases to carry on the running of a hospital shall, within twenty-one days of such cessation, surrender the certificate of registration to the Government of Uttar Pradesh in person or by registered post, and shall inform it in Form 29 of the manner in which every record held by him has been disposed of.
