Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the insured person shall apply to the Regional Board in Form 6 for a duplicate, accompanied by a fee of one hundred rupees, and the Regional Board shall issue a duplicate certificate marked as such within ten days of the application.
