Application for registration.—Every building worker registered with the Board carrying on registration with the Welfare Board shall make an application for registration to the Employees' Insurance Court in Form 12, within sixty days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every benefit held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
