Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the establishment employing persons with disabilities shall apply to the Commission in Form III for a duplicate, accompanied by a fee of one hundred rupees, and the Commission shall issue a duplicate certificate marked as such within ninety days of the application.
