Application for registration.—Every establishment employing persons with disabilities carrying on the employment of persons with disabilities shall make an application for registration to the Commission in Form V, within twenty-one days of the commencement of these rules, accompanied by a fee of one hundred rupees and a statement of every assistive device held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
