Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the unorganised worker shall apply to the Employees' Insurance Court in Form 12 for a duplicate, accompanied by a fee of fifty rupees, and the Employees' Insurance Court shall issue a duplicate certificate marked as such within thirty days of the application.
