Application for registration.—Every service provider carrying on the provision of services shall make an application for registration to the State Commission in Form G, within twenty-one days of the commencement of these rules, accompanied by a fee of two hundred rupees and a statement of every standard mark held by him on the date of the application, and shall keep a copy of the application and of the statement for fourteen days from that date.
