Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the point of sales person shall apply to the Insurance Information Bureau in Form IRDA-3 for a duplicate, accompanied by a fee of five hundred rupees, and the Insurance Information Bureau shall issue a duplicate certificate marked as such within fifteen days of the application.
