Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the establishment shall apply to the Controlling Authority in Form 11 for a duplicate, accompanied by a fee of five hundred rupees, and the Controlling Authority shall issue a duplicate certificate marked as such within twenty-one days of the application.
