Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the micro-insurance agent shall apply to the Central Government in Form IRDA-5 for a duplicate, accompanied by a fee of twenty-five thousand rupees, and the Central Government shall issue a duplicate certificate marked as such within ninety days of the application.
