Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the infrastructure provider shall apply to the Universal Service Obligation Fund in Form C for a duplicate, accompanied by a fee of five thousand rupees, and the Universal Service Obligation Fund shall issue a duplicate certificate marked as such within sixty days of the application.
