Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the employee shall apply to the Welfare Board in Form 16 for a duplicate, accompanied by a fee of five hundred rupees, and the Welfare Board shall issue a duplicate certificate marked as such within ten days of the application.
