Surrender of certificate.—Every rehabilitation professional who ceases to carry on the practice of a rehabilitation profession shall, within seven days of such cessation, surrender the certificate of registration to the Government of West Bengal in person or by registered post, and shall inform it in Form II of the manner in which every identity card held by him has been disposed of.
