Application for registration.—Every rehabilitation professional carrying on the practice of a rehabilitation profession shall make an application for registration to the Government of West Bengal in Form V, within fifteen days of the commencement of these rules, accompanied by a fee of ten rupees and a statement of every facility held by him on the date of the application, and shall keep a copy of the application and of the statement for ninety days from that date.
