Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the rehabilitation professional shall apply to the Government of West Bengal in Form VI for a duplicate, accompanied by a fee of fifty rupees, and the Government of West Bengal shall issue a duplicate certificate marked as such within seven days of the application.
