Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the licensee shall apply to the Director of Health Services in Form 21 for a duplicate, accompanied by a fee of two hundred and fifty rupees, and the Director of Health Services shall issue a duplicate certificate marked as such within fourteen days of the application.
