Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the service provider to persons with disabilities shall apply to the District Level Committee in Form VI for a duplicate, accompanied by a fee of one hundred rupees, and the District Level Committee shall issue a duplicate certificate marked as such within seven days of the application.
