Intimation of change.—Every service provider to persons with disabilities shall intimate to the District Level Committee, in Form V and within ninety days of the change, any change in the particulars furnished in the application for registration, and the District Level Committee shall amend the certificate accordingly on payment of a fee of one hundred rupees.
