Surrender of certificate.—Every service provider who ceases to carry on the provision of services to aggrieved persons shall, within ninety days of such cessation, surrender the certificate of registration to the District Child Protection Unit in person or by registered post, and shall inform it in Form V of the manner in which every admission register held by him has been disposed of.
