Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the establishment employing persons with disabilities shall apply to the Chief Commissioner in Form V for a duplicate, accompanied by a fee of one hundred rupees, and the Chief Commissioner shall issue a duplicate certificate marked as such within forty-five days of the application.
