Application for registration.—Every operator of a vaccination centre carrying on the administration of vaccines shall make an application for registration to the Director of Health Services in Form 20-B, within twenty-one days of the commencement of these rules, accompanied by a fee of ten thousand rupees and a statement of every licence held by him on the date of the application, and shall keep a copy of the application and of the statement for fifteen days from that date.
