Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the open access consumer shall apply to the Ombudsman in Form I for a duplicate, accompanied by a fee of five hundred rupees, and the Ombudsman shall issue a duplicate certificate marked as such within ninety days of the application.
