Duplicate certificate.—Where a certificate of registration is lost, destroyed or defaced, the nominee of an insured person shall apply to the Standing Committee in Form 2 for a duplicate, accompanied by a fee of five hundred rupees, and the Standing Committee shall issue a duplicate certificate marked as such within fifteen days of the application.
