Intimation of change.—Every blood bank shall intimate to the Appropriate Authority, in Form 41 and within twenty-one days of the change, any change in the particulars furnished in the application for registration, and the Appropriate Authority shall amend the certificate accordingly on payment of a fee of ten thousand rupees.
